CHC Waiver Participants and their workers should contact Tempus Unlimited, Inc. regarding the July 1, 2022 transition. Please go to Tempus’ website, https://pa.tempusunlimited.org/, for more information or contact Tempus using the information listed below.
It is important that Direct Care Workers to CHC Waiver Participants submit their shifts for their participant’s final pay periods on-time in order to be paid. Please reference the table below for the last pay periods for CHC Waiver Participants.
To ensure a smooth transition, please make it a priority to attend a Tempus EVV training, provide your email address to Tempus, and return all Transition Packets received from Tempus back to Tempus.
PPL has been informed that if Transition Packets are not on file at Tempus by June 30th, DCW paychecks may be delayed.

PPL remains the FMS Vendor for OBRA and ACT 150 Participants.
PPL Transition Checkpoints and Due Dates
Last date to submit Direct Care Worker Paperwork (paperwork received after this date will NOT be processed): 5/20/22
Date Participant and Direct Care Worker Paperwork must be submitted to Tempus: 5/21/22
Last date to submit shifts for the 5/22/22 - 6/4/22 pay period: 6/6/22
Last date to submit shifts for the 5/29/22 - 6/11/22 pay period: 6/13/22
Last Date to submit and approve shifts for prior pay periods with PPL. DCWs must submit shifts by 9pm EST, and CLEs must approve shifts by 10pm EST. (After this date, PPL will not be able to pay.): 6/24/22
